How to get Extension field availability in Report, Ipad & Iphone?

Hi All,

I have created an extension field using Adapt for opportunity and its available in opportunity header. 

1. I want to add the extension field to the following new opportunity create screen.  How to get it?

2. How to make this field available in reports?

3. How to make this field available in Ipad and Iphone?

Please help me with your inputs.
